About Near & Dear Care Connect

Why Near & Dear Care Connect Exists

There comes a point when caring about someone isn't enough.

You may love your parent, your spouse, your adult child, or another person who depends on you. You want to be there. You want to know what is happening. You want to make sure appointments are scheduled, questions are answered, important details aren't missed, and someone is paying attention.

But life doesn't always make being there possible.

You may live hours away. You may have a demanding career, children of your own, or responsibilities that compete for your attention. You may find yourself trying to coordinate care between phone calls, appointments, providers, transportation, paperwork, and family members—while wondering whether something important is falling through the cracks.

Near & Dear Care Connect was created for that space between loving someone and being physically present.

Our purpose is simple: to help families stay connected, informed, and supported when they cannot be there themselves.

We don't replace family.
We don't replace healthcare providers.
We don't make medical decisions.

We help make sure the important details don't get lost between them.

Why We Created Near & Dear

Near & Dear was created from a simple understanding: sometimes the greatest need isn't another medical service.

It's connection.

Families often don't need someone to take over. They need someone dependable who can help organize the moving pieces, communicate clearly, follow through on agreed-upon tasks, and provide a reliable point of connection.

That's where we come in.

Whether it is coordinating an appointment, helping arrange transportation, checking in, organizing information, communicating updates to authorized family members, or helping a senior navigate everyday technology, our role is to make the process feel less overwhelming.

Because peace of mind doesn't come from knowing everything is perfect.

It comes from knowing someone is paying attention.

Our Philosophy

At Near & Dear, we believe care should be personal, respectful, proactive, and human.

We believe older adults and individuals who need additional support should be treated with dignity—not managed like a task list.

We believe families deserve clear communication rather than uncertainty.

We believe small details matter.

A missed phone call can matter.

An appointment reminder can matter.

Knowing that transportation is arranged can matter.

A family member receiving an update at the right time can matter.

And sometimes, simply knowing that someone checked in can make all the difference.

Our philosophy is built around one question:

“If this were someone I loved, what would I want someone to pay attention to?”

That question guides how we serve.

Our experience has taught us that effective support isn't simply about completing a task. It's about listening carefully, understanding what matters to the individual and family, following through, documenting important information, and communicating appropriately.

We bring that same mindset to every client relationship.

You aren't simply hiring someone to make a phone call or schedule an appointment.

You're choosing someone to become a trusted extension of your support system.

Distance shouldn't mean disconnection.

Today, families are often spread across cities, states, and even the country. Being unable to physically accompany someone to every appointment or check in every week doesn't mean you don't care.

Today, families are often spread across cities, states, and even the country. Being unable to physically accompany someone to every appointment or check in every week doesn't mean you don't care.

But distance can make coordination difficult.

Remote care coordination creates a bridge.

Through phone, video, email, technology assistance, scheduling, information organization, and regular communication,

Near & Dear helps families remain involved—even when they cannot physically be present.

We bring the connection to you.

Our remote model also means support can be integrated into everyday life without requiring unnecessary travel or disrupting an individual's familiar environment.

Our Commitment to Communication

Why Remote Coordination Matters

Our Commitment to Communication

Communication is at the heart of what we do.

Families should not have to wonder:

  • Did the appointment get scheduled?
  • Did someone follow up?
  • Is transportation arranged?
  • Did Mom receive the reminder?
  • What happened at the appointment?
  • Is there something I need to know?

  • Did the appointment get scheduled?
  • Did someone follow up?
  • Is transportation arranged?
  • Did Mom receive the reminder?
  • What happened at the appointment?
  • Is there something I need to know?

With appropriate authorization, we keep designated family members informed about agreed-upon activities, updates, and concerns.

We believe communication should be:

Clear. Timely. Respectful. Consistent.

And when something falls outside our role or raises a concern that requires professional attention, we believe in saying so.

We will never pretend to know something we don't know.

We will never make a medical decision simply because it would be easier.

Trust is built by communicating honestly—even when the answer is, “This needs to go to the right professional.”

Our Non-Medical Boundaries

The Near & Dear Promise

The Near & Dear Promise

Trust also comes from knowing what we don't do.

Near & Dear Care Connect provides non-medical care coordination and family support services.

We do not diagnose medical conditions.

We do not provide medical treatment.

We do not prescribe, administer, or change medications.

We do not provide medical advice.

We do not replace physicians, nurses, therapists, home health agencies, emergency services, or other licensed healthcare professionals.

Instead, we work alongside the existing support system—helping with communication, coordination, organization, scheduling, technology assistance, resource navigation, and other non-medical support within the scope of our services.

And when something requires medical expertise, we know when to step back and direct the situation to the appropriate professional.

That boundary isn't a limitation.

It's part of our commitment to responsible care.

The Near & Dear Promise

The Near & Dear Promise

The Near & Dear Promise

We know that trusting someone with the wellbeing of a person you love is not a small decision.

You aren't simply choosing a service.

You're choosing someone to be part of your support network.

That's why Near & Dear Care Connect is built around three things:

Connection.
Keeping families and individuals connected when distance or circumstances make it difficult.

Coordination.
Helping organize the details so important tasks, appointments, information, and follow-ups don't become overwhelming.

Communication.
Making sure the right people have the right information at the right time.

We can't promise that every situation will be easy.

We can't promise that nothing unexpected will happen.

But we can promise to approach every client relationship with professionalism, respect, clear communication, and genuine care.

Because when someone you love needs support, you shouldn't have to choose between being there and living your own life.

Near & Dear Care Connect helps bridge that gap.

Remote Support. Real Connection.
